The AZ DES Family Assistance Administration has a “client liaison unit.” It would make sense, from the name, that the purpose of this group is to work with the FAA clients — which includes anyone who is applying for, or receiving, nutrition, cash, or medical assistance in Arizona.

On Wednesday, July 29, 2026, I sent an email inquiry, asking what was the procedure for logging a formal complaint. It took 12 days (which includes two weekends) to receive a response. I was told that complaints should include the (1) case name, (2) case number, (3) date of birth, and (4) telephone number and could be sent to their email address, which is faaclientliaisonunit@azdes.gov.

Yesterday, August 13, 2026, I replied with my formal complaint, outlining the basic facts and also including the remedies that I am seeking. Asking for specific remedies was, actually, a suggestion from AI. It seemed like a good idea. I’m not sure whether or not my requests will go anywhere, but I was curious to see how the FAA Client Liaison would respond.

In my own situation, the problems I enumerated included:

  • losing SNAP for a family member because FAA had not processed documents that had been submitted on time
  • losing SNAP for a family member because FAA ignored determinations from a few months prior that had been grounded in submitted documents
  • failure to process SNAP appeals (60-day federal time frame)
  • failure to process SNAP change reports (10-day time frame according to policy manual for changes that cause increase in SNAP, benefits to increase the following month)
  • obvious misinterpretation of information provided in change report
  • Scheduling termination of SNAP case due to FAA not reviewing submitted documents.

The remediations that I requested included the following:

  • A tracking number for the complaint
  • determining the disposition of faxed appeals
  • reviewing and processing change reports
  • a written response identifying what corrective action is being taken.

When I get a response, I will update this post with the results.

For anyone wishing to send their own complaint, this would be the basic format:

Section 1

Case Name:

Case Number:

Date of Birth:

Telephone Number:

Section 2

List each problem in your case as a separate paragraph. If you know the policies/laws/rules that are not being followed, point this out. Keep it brief, with a minimum of details. Stick to the facts, not the emotions. My recommendation would be to list them in date chronological order.

Section 3 (optional)

List any requests that you may have, any desired responses, a tracking number for the complaint, etc.
